Medicare coverage is the determination that an item or service falls within a Medicare benefit category and meets applicable coverage rules. Coverage may be set through a national coverage determination by the Centers for Medicare & Medicaid Services, a local coverage determination by a Medicare contractor or another applicable coverage process.

Coverage is not the same as FDA authorization. An implantable device may be approved for marketing yet still require separate evidence that it is reasonable and necessary for Medicare patients. Coverage can also be limited to certain diagnoses, clinical settings or studies. Coverage with evidence development can link payment to participation in specified evidence-gathering arrangements.

For a brain-computer interface, practical access depends on much more than the implant's regulatory status. Device costs, implantation, follow-up visits, training, software, maintenance and rehabilitation can all affect care delivery and may be treated differently by coverage and billing rules.

Do not assume that a promising BCI trial, Breakthrough Device designation or even an eventual FDA approval means Medicare will pay for the procedure. Check the relevant CMS or contractor policy, eligibility rules and out-of-pocket responsibilities for the specific device and indication.
